JOB SUMMARY:
The Board-Certified Music Therapist is responsible to provide music therapy services to Peaceful Hearts Hospice, LLC patients under the supervision of the Director of Holistic Services. The Board-Certified Music Therapist is responsible for following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team (IDT) to help meet patient care outcomes.
Organization Relationship:
The Board-Certified Music Therapist reports directly to the Director of Holistic Services.
Qualifications:
Education and Experience:
- Must be a Board-Certified Music Therapist.
- Must have at least one (1) year experience as a Board-Certified Music Therapist.
- Experience of one (1) year in hospice, home health or oncology, preferred.
Responsibilities and Duties:
- Provide music therapy services for patients according to the directions of the Director of Holistic Services and in accordance with the plan of care.
- Assess patients for appropriate medical indication for services.
- Treat patients to relieve symptoms and provide physical, emotional, mental, and spiritual support and comfort.
- Evaluate treatment outcomes and recommend new or altered treatments as necessary to further promote, restore, or maintain health, in collaboration with the Director of Holistic Services.
- Collect medical histories and general health and lifestyle information from patients.
- Report all patient change in condition and response to interventions and/or symptom concerns to the Registered Nurse Case Manager (RNCM) and the Director of Holistic Services within a timely manner.
- Update other disciplines involved in patient’s care, as needed, ensuring a team approach to caring for all aspects of patient needs (social work, dietician, hospice aide, chaplain, volunteers, and holistic).
- Communicate any situation where staff is unable to meet a patient’s needs to the Director of Holistic Services.
- Provide care in all settings, including but not limited to skilled nursing facility, assisted living, independent living, or any home setting.
- Provide emotional support at time of death to family and caregivers.

Pay: $25.00 - $35.00 per hour
