Position Summary:
The Hospice Music Therapist provides individualized music - based interventions to address physical, emotional cognitive, and social needs of hospice patients. You will collaborate with the interdisciplinary team to enhance quality of life, reduce anxiety, and create meaningful end-of-life experiences.
Key Responsibilities:
· Assess patient needs and develop individualized music therapy care plans.
· Provide music interventions (live music, singing, guided relaxation, legacy projects, etc)
· Address symptoms such as anxiety, pain, agitation, and emotional distress
· Adapt sessions based on patient condition and responsiveness
· Engage families in music experiences when appropriate
· Provide emotional support and meaningful connection opportunities
· Assist with memory-making and legacy activities
· Participate in IDG (Interdisciplinary Group) meetings
· Coordinate with nurses, social workers, chaplains, and aides
· Communicate patient response and progress to the team
· Complete timely documentation in EMR
· Maintain care plans and visit notes per hospice regulations
· Ensure compliance with Medicare Conditions of Participation
Qualifications:
· Bachelor's degree in Music Therapy
· Board Certified Music Therapist (MT-BC) or eligible
· Strong interpersonal and communication skills
· Ability to work with diverse population and diagnoses
· Valid driver's
· license and reliable transportation
Preferred:
· Hospice or healthcare experience
· Experience working with geriatric or terminally ill patients
· Familiarity with EMR systems (HCHB preferred)

Travel required within assigned service area
Pay: $50,000.00 - $65,000.00 per year
